Mission Statement: There are a multitude of resources available to assist you in meeting your caregiver needs and your elder in meeting their individual needs, both in the home and in an out of home placement. Exploring and locating these resources can be overwhelming and time consuming. My mission is to provide you with an individualized resource list from our consultation meeting to guide you and your elder family member (no cost, low cost and fee for service resources). This offers you and your family the gift of time to meet your own needs and spend quality time together. In addition, I offer counseling, support group, and family meeting sessions, as well as other ancillary services to support and guide you in your journey.
Option A: “Where Do I Start?” Consultation, 75 min. Meeting
- Meet for one 75 minute consultation to explore yours and your elder family member's needs at this time, as well as future services and issues you will need to consider (You will receive a checklist titled: Important Issues to Consider When Caring For Your Elder Family Member) at this consultation. Elders are welcome and encouraged to attend all meetings.
- You will receive a listing of the general community services available at this consultation meeting (no fee and low fee services).

You will receive Tip Sheets with topics on a variety of issues, such as: Twelve Ways to Reduce Caregiver Stress, How to Get Your Elder to Accept Help, Recognizing the Telltale Signs (that your elder needs assistance), Determining When You Need a Family Conference, Monitoring and Evaluating Your Elder’s Care, Common & Dangerous Misperceptions About Aging.

You will receive educational materials regarding dementia, Alzheimer's, depression and anxiety – common issues in the elder population.
- Advance Directives, and other resources.
Option B: “Now What? Where? When? & How?” Consultation
Includes all Option "A" items, with the addition of the following:

I will prepare and present an individualized package for you listing the resources in your community, based on your specific needs (no fee, low fee & fee for service options), as well as guide you in prioritizing your needs.
- You will receive two 30 minute phone calls providing you with emotional support and guidance.
Option C: “Simplify, Engage & Solutions” Plan
Includes Options "A" and "B" items, with the addition of the following:

Two 50 minute home visits to provide emotional support/guidance to you, the caregiver or elder (with your elder’s permission only, if for them).
- On yours and your elder’s behalf, I will organize and host one family meeting to assist in the discussion and negotiation of group effort in caring, locating, & arranging services for your family member (one 2 hour meeting
- Individual Counseling Services (you will receive one 50 minute counseling session for you and/or your spouse or partner).
